2007-2008 Lunch Order Form

This year Destiny Community School is offering a lunch program provided by Preferred Meal Systems.  Preferred Meal Systems is an approved food service company by the Arizona Department of Education.  As such, they provide nutritious meals to students that are both tasty and healthy.  Lunch will be provided 5 days each week and will include milk.  The menu will change daily offering students variety in their lunch meals. 

The cost is $2.50 per meal for a total of $110.00 per child for the entire quarter.

The following rules apply to our lunch program:

1. Orders are pre-paid  by the quarter only.  No weekly or monthly payments will be accepted.
2. There are no refunds or rain checks if your student is absent.
3. There is no splitting of orders such as Wednesdays only.
4. Late payment of orders will not be accepted.
